Based on observation and interview with facility staff the facility failed to ensure guard rails in exit stair are poperly installed. In the event of an emergency all staff and clients using the stairs may be affected.

Findings Include:

On 06/17/2015 between 10:00 a.m. and 1:00 p.m. observation revealed guard rails at interior stairs have openings larger than those that a 6 inch sphere could pass through.

Based on observation and interview with facility staff the facility failed to ensure total sprinkler coverage in all areas. In the event of a fire in the affected areas all staff and clients may be affected.

Findings Include:

On 06/17/2015 between 11:30 a.m. and 12:30 p.m. observation revealed the area under the HVAC duct in the mechanical room that is greater than 4 feet in width and the Main Employee Entrance overhang, which is greater than 4 feet wide and combustible, are not sprinkler protected underneath.

Findings Include:

On 06/17/2015 at approximately 11:50 a.m. observation revealed the kitchen hood suppression system does not completely cover the range, the broiler, and the griddle.
