Based on review of 4 swing bed clinical records and interview with staff, 2 of 4 patient swingbed records failed to have a comprehensive activity assessment for these patients.
Findings were:
Review of the swingbed records for Patient #1 and Patient #2 revealed that these patients failed to have any activity assessment provided by the Activity Director. Interview with the CEO in the workroom on 3/21/12 confirmed the above findings.

Findings were:
Review of the 4 swingbed records revealed that there had been no history and physicals completed for 4 of 4 patients. Interview with the CEO in the workroom on 3/21/2012 confirmed these findings.
