Based on observation and staff interview the facility failed to maintain the 90 minute fire door within the horizontal exiting. This deficient practice would allow smoke and fire to migrate throughout each compartment affecting all residents, staff, and visitors in two of seven smoke compartments. Facility census was 90 residents.
Findings are:
Observation on 6-17-15 at 1:20 pm revealed the horizontal exiting doors next to outpatient failed to latch when closed.
During an interview on 6-17-15 at 1:20 pm, observation was acknowledge and verified by Maintenance A.
NFPA Standard:
NFPA 101, 2000 ed., 19.2.2.5. Horizontal exits complying with 7.2.4 and the modifications of 19.2.2.5.1 through 19.2.2.5.4 shall be permitted.
