PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on clinical record reviews and interviews with facility personnel for one of one sampled patients (Patient #6), the facility failed to ensure that a physician order was completed with the initiation of restraints.
The findings include:
1. Patient #6 was admitted to the ED on 5/9/11 with alcohol intoxication. Review of the ED record dated 5/9/11 identified that the patient was placed in 4-point restraints for three hours due to threatening and agitation. Further review failed to identify that a physician's order and/or a one to one evaluation was completed with the initiation of restraints. Review of hospital policy identified that the physician order is written on the restraint order form including a face to face evaluation of the patient within one hour of the application of restraints. Interview with the ED Nurse Manager on 6/8/11 identified that there was no physician order and/or one to one evaluation by a physician.
