Based on record review, document review and staff interviews it was determined the facility failed to make appropriate arrangements for discharge planning in one (1) of ten (10) records reviewed (Patient #1). This failure has the potential to affect all of the patients in the facility.

Findings include:

1. A medical record review of Patient #1 revealed she was an inpatient from 9/13/17 through 9/16/17. Her diagnosis on discharge included a fracture of the left tibia.

2. On discharge, the physicians determined Patient #1 required a walker for ambulation in the home.

3. The facility failed to provide Patient #1 with a walker at discharge.

4. Patient #1 was discharged home without a walker.

5. In a joint interview, the Manager of case management and the Director of case management both concurred Patient #1 was discharged home, without a walker, which was an unsafe situation.
