Based on observation and interview, the facility failed to ensure the fire doors on the west wing would latch properly upon activation per NFPA 101 Life Safety Code, 2012 Edition, Section 19.3.7.8. The deficiency affected one of four smoke compartments in the building.
Findings include:
During an observation on 11/28/16 at 3:10 p.m., the smoke barrier doors on the west wing were exercised. The pair of 1.5 hour fire rated doors failed to close completely after being activated.
During an interview on 11/28/16 at 3:11 p.m., staff member A stated that the south most door could be sped up to increase the swing so it would close properly. He agreed that the door failed to close properly after several tries.
