Based on observation and interview the facility failed to provide clean and sanitary environment for patients in the recovery room on 3 of 3 days.
The findings included:
Observation in the recovery room on 3 of 3 days revealed a strong foul unclean odor that remained throughout the survey.
Interview on 12/8/15 at 1:00 PM, with the Director of Nursing (DON) stated she would have maintenance check the room for the odor. On 12/9/15, third day of the survey the DON stated, maintenance thought it was the sink drain related to infrequency of use and other drains were connected to the sink.
