Based on observation and interview, the facility had a corridor door that did not meet the requirements of NFPA 101 LSC (00) Section 18.3.6.3.6. This deficient practice could affect the safety of all patients, staff and visitors, if smoke from a fire were allowed to enter the exit access corridors making it untenable.

Findings include:

On facility tour between 9:30 AM and 12:30 PM on 07/01/2014, it was observed that there were wedges found in the in Laboratory by the door that opens to the corridor. It was verified that the wedges are being used to prop the Laboratory door open during moving supplies. This condition results in the use of wedges as unapproved door hold open devices.

This deficient practice was verified by the Maintenance Manager (RK).

Based on observations and staff interview, it was revealed that the facility has failed to provide proper protection from 2 of several hazardous areas located throughout the facility in accordance with NFPA Life Safety Code 101 (2000 edition) section 19.3.2.1. This deficient conditions could in the event of a fire, allow smoke and flames to spread throughout the effected corridors and areas making them untenable, which could negatively affect the exiting capabilities for 5 of 25 patients, staff and visitors.



Findings include:

On facility tour between 9:30 AM and 12:30 PM on 07/01/2014, observation revealed, that the storage room door in the west wing and the soiled linen room across from the main nursing station did not positively close and latch into the door frames.


This deficient practice was verified by the Maintenance Manager (RK).
