Based on observation during the survey walk-through, staff interview, and document review during the Life Safety portion of a Critical Access Hospital Recertification Survey conducted on March 4, 2019, the surveyor finds that the facility failed to provide and maintain a safe environment for patients and staff.

Based on observation, document review, and staff interview, it was determined for 3 of 3 weight scales in the Emergency Department (ED), the Critical Access Hospital (CAH) failed to ensure scales were calibrated for accuracy. This has the potential to affect all patients receiving care in the Emergency Department (ED) (approximately 10 patients per day).

1. During a tour of the ED on 02/25/19 at 12:45 PM, the following was observed: 2 standing scales and 1 infant scale without proof of calibration. There were preventive maintenance stickers on the scales indicating they were calibrated in October 2017.

2. On 02/27/19 at 11:30 AM, a request was made to E #1 for a CAH policy on scale calibration. E #1 provided a "Biomedical Equipment Management Plan", revised 03/05/14, which included a list of equipment that required calibration and had been calibrated but there was no requirement to calibrate the weight scales or history of any scale being calibrated. There was no policy on scale calibration.

3. On 02/25/19 at 1:00 PM, an interview with the Chief Executive Officer (E #1) was conducted. E #1 confirmed the 3 scales had not had the proper preventive maintenance or calibration performed and indicated they should have been calibrated in October 2018.
