Based on observations and interviews the facility failed to ensure fire and smoke rated doors operated in a manner that would resist the passage of smoke and/or fire. This failed practice placed occupants in 2 out of 6 smoke compartments at risk for exposure to smoke/fire environment. Findings:
Observation on 6/23/16 at 10:45 am revealed a fire rated door protecting the sky bridge did not close and latch.
Observation on 6/23/16 at 11:00 am revealed a fire rated door protecting the corridor outside of obstetrics did not close and latch.
Observation on 6/23/16 at 11:30 am revealed a fire rated door protecting the corridor outside of surgical services did not close and latch.
The Safety Officer acknowledged and confirmed the findings at the time of their discovery. The doors were adjusted after the survey and were in compliance at the time of exit.
