Based on record review and interview it was determined that the hospital did not include a written list of agencies that were available to discharging patients that were in need of post hospital extended care services.

Findings included:
On 1/8/13 and on 1/14/13, a total of 11 patient charts were reviewed. The patient charts reviewed were patients 1 through 11. There was no written list of agencies that were possibly available to the patients upon discharge that were in need of post hospital extended care services.
On 1/8/13 and on 1/14/13, a total of 6 patient interviews were completed. Patients 12 through 17 were interviewed. The patients were asked if they had received a list of post hospital care service agencies (such as nursing homes or home health agencies) to review before their impending discharge. Each of the patients stated no.
On 1/14/13, the Case Management Director was interviewed. She stated that sometimes lists of post hospital care service agencies were given to the patients, however the patients never sign them nor are the lists made part of the patients' medical charts.
