Based on record review of 21 closed medical records and 3 open, it was determined that the closed medical records for three Medicare recipient patients were missing the Important Message (IM) from Medicare.
Patient #1 was admitted for 14 days and had initial IM but was missing the second one. Patient #2 was admitted for 10 days and was missing the IM that is to be given within 2 days before discharge. Patient #3 was admitted for 6 days and was missing both the first and second IM.
According to policy #CM1003, "Medicare Important Messages," the admitting department is supposed to deliver the first IM and case management is tasked with delivering the second IM.
