Based on interview and document review, the facility did not ensure that all staff who monitor patients in restraint or seclusion receive training in first aid. In 1 of 1 Nurse Aide's personnel file (Staff A) reviewed, it lacked evidence of training in first aid. This could place patients at risk for potential harm.


Findings include:

-- Per interview of Staff A on 8/9/2022 at 10:30 am, he/she monitors patients in restraints. Staff A indicated he/she did not have any first aid training.

-- Per review of Staff A's personnel file (hire date 3/10/14), it lacked evidence of first aid training.

-- Per interview of Staff B on 8/11/22 at 9:55 am, he/she confirmed Staff A did not have documentation of first aid training in his/her personnel file.
