Based on medical record review, it was determined the facility to maintain medical records in a manner that was legible for 2 (Patient #23 and 26) 28 records of sampled patients reviewed.
The findings included:
1. Medical record review for Patient #23 revealed an admission date of 5/5/10 with diagnosis which included depression. Review of a physician's order dated 5/8/10 revealed an order that was scribbled thru and was unreadable.
2. Medical record review for Patient #26 revealed an admission date of 4/23/10 with diagnoses which included infected PEG [Percutaneous Endoscopic Gastrostomy] and delusions. Review of a physician's order dated 5/17/10 revealed an order that was scribbled thru and unreadable.

Based on observation and interview it was determined the facility failed to have technical personnel competent to maintain a clean and sanitary kitchen.
The findings included:
1. Observation, during the initial tour of the kitchen on 5/17/10 at 2:30 PM, revealed a build up of grease on the hood of the cook top stove.
Observation of the ice machine vent revealed a build up of dust and observation of standing oven revealed a large build up of a black substance on 2 shelves of the oven.
2. During an interview, the Director of Dietary (DOD) present at the time of the above observations, stated the cook top stove hood was cleaned by a contracted company once a quarter and had last been cleaned by the company on 2/11/10. The company was scheduled to return the week of 5/10/10.
The DOD confirmed the build up of dust should not be on the ice machine vent and stated the large build up of black substance on the shelves in the standing oven was "baked on grease and run over from cobbler."
