Based on observation during tour it was determined this facility failed to ensure the exit discharge was arranged so that failure of any single lighting fixture will not leave the area in darkness. This had the potential to affect all those utilizing this facility. The facility census was eleven at the beginning of the survey.
Findings include:
Tour of the facility took place on 01-04-11 with staff members P, Q and R. During tour of the east (section B) and northeast (section C) exit discharges, observation was made of a single light fixture mounted above each exit access door. Both exit discharges was bordered by a grassy area which was lined with trees and brush. No additional lighting was observed in the area.
This finding was verified by all staff members during tour and acknowledged when staff Q stated we will have to add additional lights to these areas.
