Based on observations made during the tour of the facility on the afternoon of 08/04/10 with the hospital administrator, maintenance director, and the chief nursing officer, the facility failed to assure that hazardous areas were protected in accordance with NFPA 101, 19.3.2.1.
The door to the soiled utility room in the surgical suite was not self-closing because the closer had been disconnected.

Based on observations made during the tour of the facility on the afternoon of 08/04/10 with the hospital administrator, maintenance director, and the chief nursing officer, the facility failed to assure that the fire alarm system was in full compliance with NFPA 72.
The fire alarm control panel at the nurses' station in the nursing unit did not have a label on the face of the panel that indicated the electrical panel and circuit number that supplied power to it.

Based on observations made during the tour of the facility on the afternoon of 08/04/10 with the hospital administrator, maintenance director, and the chief nursing officer, the facility failed to assure that the automatic sprinkler system was in full compliance with NFPA 13.
The cabinet containing the sprinkler spares did not have a wrench for changing out sprinkler heads.

Based on observations made during the tour of the facility on the morning and afternoon of 08/04/10 with the hospital administrator, maintenance director, and the chief nursing officer, the facility failed to assure that the Type I essential electrical system was in full compliance with NFPA 99, 3.4.2.2.2.
(1) The medication refrigerator in the emergency department was plugged into a normal power receptacle instead of an emergency receptacle.
(2) Emergency treatment room 5A did not have any lighting on emergency power.
(3)The autoclave in the sterile processing room did not appear to be plugged into an emergency receptacle.
(4) The hospital pharmacy did not appear to have any lighting on emergency power.
(5) There did not appear to be any receptacles and lighting in the patient rooms, because they were not color coded.
(6) The emergency generator shed did not have an emergency receptacle. It also did not have any lighting, with battery backup, on emergency power.
(7) The fire alarm system was listed on the directory of the critical branch panel located adjacent to the nurses' station serving the nursing unit, instead of from a life safety branch panel.

The emergency receptacles in the emergency department treatment rooms did not have permanent labels to identify which electrical panel bords and circuits supplied power to them.
