PATIENT RIGHTS: GRIEVANCES

Based on document review and staff interview, it was determined the hospital failed to provide patients or patients' representatives with the phone number and address for lodging a grievance with the State Agency. This failure has the potential to affect all patients in the hospital.

Findings include:

Review of the hospital's admission packet revealed no documented evidence the West Virginia State Agency address and phone number were included.

An interview was conducted with the Director of Intake on 7/26/22 at 2:00 p.m. They concurred the West Virginia State Agency address and phone number were not included in the admission packet.

PATIENT RIGHTS: CARE IN SAFE SETTING

Based on medical record review and staff interview, it was determined the hospital failed to provide a safe environment. A patient retrieved a Vape Pen that was unsecured at the second floor nurse's station. This failure was identified in one (1) of one (1) patients (patient #7) and has the potential to harm all patients.

Findings include:

Review of the medical record for patient #7 revealed a nurse's note from 7/11/22 at 10:02 p.m. that stated in part: "[Patient #7] at approximately 8:23 PM, saw a Vape Pen charging behind the Nurses' Station and while the Staff remained busy dealing with another situation on the A Hall side of the Unit, reached through the window on the B Hall side of the Nurses' Station and [he/she] was able to take hold of the Vape Pen and bring it out from the Nurses' Station and [he/she] did take some vapes off of it and then passed it on to a [male/female] co-patient, who also vaped from it. Later at approximately 8:50 PM, [patient #7] was in the bathroom on the A Hall side of the Unit. One of the BHT's [Behavioral Health Technician] reported that [he/she] had been inside this bathroom for a suspicious amount of time and this BHT expressed concern that [patient #7] may be in the bathroom self harming, for another Patient had just self harmed. I did knock on the Bathroom door and [patient #7], in a rather cross sounding tone, asked what I wanted. I asked [him/her] if [he/she] was doing alright and [patient #7] yelled, "I'm taking a shit." [He/she] came out shortly after and the BHT working the Unit went into the bathroom right away and [he/she] found a Vape Pen behind the shower curtain."

An interview was conducted with the Nurse Manager of 2 East and 2 West on 7/25/22 at 4:15 p.m. They concurred patient #7 retrieved a Vape Pen that was unsecured at the second floor nurse's station.
