Based on observations, the facility failed to maintain the required 2 hour separation between the healthcare facility and a business occupancy.
The findings included:
Observation on 6/20/17 at 10:05 AM, revealed no fire stop systems were used on the penetrations located throughout the 2 hour fire wall located between the healthcare facility and the business occupancy. NFPA 101, 19.1.3.1 (2012 Edition), NFPA 101, 6.1.14 (2012 Edition), NFPA, 101, 6.1.14.4.1 (2012 Edition), NFPA 101, Table 6.1.14.4.1(b) (2012 Edition), NFPA 101, 8.3.5 (2012 Edition), and NFPA 101, 8.3.5.1 (2012 Edition)
The maintenance director was present when these deficiencies were identified and the administrator acknowledged these deficiencies during the exit conference on 6/13/17.
