Based on observation and interview the facility failed to properly store sterilization supplies in the sterilization processing room. These practices could lead to contamination and possible infection of patients.
On tour of the facility on 2/6/2013 it was observed in the sterilization processing room stored on the shelf a box of gloves which had expired 12/2012 and the chemical steam indicators being placed in the instrument packs had expired 01/2003.
An interview with staff #1 and staff #2 on 2/6/2013 at 2:30 PM confirmed the supplies had expired and were still being used in the sterilization process of supplies and instruments.

Based on observation, record review, and interview the facility failed to monitor the temperature and humidity in the sterilization processing room.
On tour of the facility on 2/6/2013 it was observed in the sterilization processing room there were no gauges to monitor the temperature or humidity and observed an open area in the ceiling of the sterilization processing room. The cover for the ventilation return was missing.
A review of records revealed temperature and humidity were not being monitoring in the sterilization processing room.
An interview with staff #1 and staff #2 on 2/6/2013 at 2:30 PM confirmed the facility has not been monitoring the temperature and humidity in the sterilization processing room.
