Based on observations and interview, the facility failed to maintain the physical plant.
The findings included:
During an interview on 10/20/16 at 10:30 AM, the facility maintenance director revealed windows had been replaced by a contractor that had not come back. Observation of the following rooms and areas revealed moisture damage and black mold around the top of the windows spreading to the walls.

e. the ground floor of the stairway had black stains on the floor around the walls.
f. soiled linen room had black mold on ceiling.
The facility director and executive director was present when the deficiencies were identified on 10/20/16.
