Based on observation and interview Registered Nurse #1 failed to consistently adhere to the standard of care for medication administration.

The Surveyor observed Registered Nurse #1 (RN #1) prepare and deliver medications to Patient #2 on Stanton 6 at 10:30 A.M. on 10/22/19. RN #1 prepared two injectable medications for patient #2. One of the medications was to be delivered by the intravenous route and one was a subcutaneous injection (an injection where a short needle is used to inject a drug into the tissue layer between the skin and the muscle). RN #1 opened both vials and drew up the medication but failed to disinfect the septum (top) of the vials before inserting the sterile devices through the septum.

The Surveyor interviewed the Nurse Manager of Stanton 6 at 11:00 A.M. on 10/22/19. The Nurse Manager of Stanton 6 said that the nurses are expected to disinfect the septum prior to using the vial.
