STANDING ORDERS FOR DRUGS

Based on document review and interview, it was determined that for 1 of 10 (Pt. #1) clinical records reviewed, the Hospital failed to ensure medications were reordered as required by policy.
.
Findings include:

1. The policy titled "Orders Prescriber (effective date 3/23/01)" was reviewed on 7/9/24. The policy noted "XII. Automatic Stop Orders. A. Medication orders are assigned an automatic stop date in the computer ordering system....Prescribers receive messaging through the computer system prior to this date."

2. The record of Pt#1 was reviewed on 7/9/24. On 12/5/23 to Registered Nurse (RN) triage note at 8:05 AM included Pt. #1 was admitted to the Emergency Department (ED)- Crisis Stabilization Unit (CSU) on 12/5/23 with Violent and Aggressive Behavior. On 2/27/24 at 10:14 AM, the RN noted pt. having seizure like activity. The medication Oxcarbazepine 600 mg was ordered on 12/6/23 to start and end date of 2/3/24 after 32 doses. Pt#1 didn't miss any doses of the Oxcarbazepine. The medication was reordered on 2/27/24. The medication Brivaracetam 50 mg was ordered on 12/6/23 to start and end date of 2/4/24 after 120 doses.

3. An interview was conducted on 7/9/24 with the Risk Manager (E#3). E#3 verbally confirmed the order wasn't renewed.
