Based on observations and staff interview, the facility failed to properly maintain secured oxygen tanks as evidenced by the unsecured oxygen tank on the floor in the Hospice area. This deficiency could affect all occupants of the facility in case of a fire or other emergency.

Findings include:
During the Life Safety Survey tour of the facility at 3:25 pm on April 10, 2012 with the Plant Operations Director, it was noticed that an unsecured oxygen cylinder was on the floor in the Hospice Area.

During the Staff Interview at 3:30 pm on April 10, 2012, the Plant Operations Director stated that the oxygen cylinder will be immediately secured.

Based on records review, staff interview, and observations, the facility failed to comply with all electrical safety requirements as evidenced by the lack of documentation for the main and feeder breaker exercise test performance and the use of an extension cord to power a water cooler. This deficiency could affect all occupants of the facility in case of a fire or other emergency.

Findings include:
During the records review process of the facility between 10:30 am and 3:30 pm on April 10, 2012 with the Plant Operations Manager, it was revealed that there was no documentation for the main and feeder breaker exercise test performance.

During the Staff Interview between 10:35 am and 3:35 pm on April 10, 2012, the Plant Operations Manager confirmed the lack of documentation for the main and feeder breaker exercise performance.

During the Life Safety tour of the facility at 2:20 pm on April 11, 2012 with the Plant Operations Director, it was revealed that an extension cord was powering a water cooler in the first floor Environmental Director's Office.

During the Staff Interview at 2:25 pm on April 11, 2012, the Plant Operations Director acknowledged the extension cord was powering a water cooler in the first floor Environmental Director's Office.
