K908 Gas and Vacuum Piped Systems - Inspection and Testing Operations

Based on visual observation the facility failed to have the Med Gas System annually inspected and maintained. The deficient practice had the potential to affect 48 of 48 residents.

Findings:

During the facility tour and records check of Sabine Medical Center on March 21, 2023 revealed that the Med Gas system was inspected on 3/31/2022 which revealed several issues with the med gas system that were not repaired.

Interview with administrator revealed the facility was not aware that the med gas report contained several deficiencies. The facility has hired a facility director in November 2022 to co ordinate fire & safety issues.

K914 Electrical Systems - Maintenance and Testing
Based on visual observation and record review, the facility failed to assure that the annual testing program on the emergency generator was conducted and documented. In cases of a power outage the emergency generator powers essential life safety equipment for the facility. The deficient practice had the potential to affect 48 of 48 residents.

Findings:

During the record review on March 21, 2023 revealed that the generators did not have an annual inspection by a certified contractor.
Interview with administrator revealed the facility was not aware that the annual inspection of the emergency generator was last performed on 12/23/2021.
