Based on observation and interview the facility failed to maintain the facility's wall, floors, equipment and supplies.
A tour of the Emergency Room was done on 12/5/2013 at approximately 1:30pm with staff #2. It was observed, the paint on the wall was noted to be chipped and flaking in multiple areas throughout the hallways. The areas where the paint was missing exposed a porous surface that would prevent cleaning and disinfecting of the walls.
In multiple areas, throughout the hallways, the base boards were pulled away from the walls, leaving an open area between the wall and base board. The open areas exposed a porous surface that would prevent cleaning and disinfecting of the walls and base bords.
At the end of one of the hallways, an area of the tile flooring was observed to be chipped and broken. Pieces of the tile flooring was missing, exposing the concrete underneath. The open areas exposed a porous surface that would prevent cleaning and disinfecting of the floors.
The tour continued to an exam room used for gynecological exams, (the physical examination of the female pelvic). A four ounce, half filled, bottle of betadine was observed sitting on a cabinet. The bottle of betadine had no makings or date as to when it was open. The bottle of betadine was available for patient use with no way to insure the integrity of the betadine.

The tour continued to the Cardiac/Trauma Room, accompanied by staff #2. Upon entry into the Cardiac/Trauma Room, the adult and pediatric crash cart (a set of trays/drawers/shelves on wheels used in hospitals for transportation and dispensing of emergency medication/equipment at site of medical/surgical emergency) were observed covered with what appeared to be the sticky remains of tape. The sticky substance would prevent cleaning and disinfecting of the crash carts.

An audit was done of the airway cart located in the Cardiac /Trauma Room. The audit revealed, 10 foil packages, labeled lubricating-jel, with the expiration date of 3/13 and approximately 15 tongue blades were observed with an expiration dates of 2005. The expired lubricating-jel and tongue blades were available for patient use.

An interview on 12/5/13 at approximately 1:00 pm, was conducted with staff #4 in the conference room. Staff#2 and staff#3were present during the interview. Staff #4 was asked to provide an overview of the infection control program. Staff #4's presentation did not provide evidence of a system for monitoring, identifying and/or reporting of areas for potential infections identified during the facility tour:
1. the half full betadine bottle in the gynecological exam room,
2. the base boards had pulled away from the walls, throughout the hallways,
3. exposing the concrete underneath the broken tile floor, located in the hallway,
4. the adult and pediatric crash cart, covered with what the sticky remains of tape,
5. 10 foil packages labeled lubricating-jel with the expiration date of 3/13 and approximately 15 tongue blades were observed with the expiration dates of 2005
