Based on observation and interview, it was determined the facility failed to maintain two hazardous area enclosures, affecting one of five floors.
Findings include:
1. Observation on January 04, 2017, between 10:22 a.m. and 10:33 a.m. revealed the following:
a. 10:22 a.m., the basement level, elevator number seven, elevator machine room door lacked a self-closing device.
b. 1033 a.m., the basement level High Voltage Room door lacked smoke-tight integrity when coupled with the corresponding door frame assembly.
Exit interview with Facilities Manager between 11:30 a.m. and 11:40 a.m. confirmed the hazardous area enclosure deficiencies.

Findings include:
1. Observation on January 04, 2017, between 10:44 a.m. and 11:05 a.m. revealed the following corridor doors lacked smoke-tight integrity:
a. 10:44 a.m., the first floor, OR area Housekeeping Closet.
b. 11:05 a.m., the second floor, Same Day Surgery Waiting Room.
Exit interview with Facilities Manager between 11:30 a.m. and 11:40 a.m. confirmed the corridor opening deficiencies.
