Based on observation on 01/05/11 of the Acadia Family Health Center, 460 Main St Madawaska, the following Life Safety Code violations are noted:

1) Where this Code requires an exit to be separated from other parts of the building, the separating construction shall meet the requirements of Section 8.2 and the following.
(a) * The separation shall have not less than a 1-hour fire resistance rating where the exit connects three stories or less.

The stairtower exit enclosure on the south side of the building does not have doors on the first floor protecting the enclosure. The doors have been removed.

2) Door wedge opening stair exit enclosure door on 3rd floor.

3) Fire alarms shall be maintained and tested in accordance with NFPA 72. There is no documentation of any fire alarm testing for the building.

4) Door leading to kitchen and protecting stairs leading to lower floor gym shall have installed a self closing and positive latching device.

Observation on 01/05/11 in the sprinkler room showed two electrical junction boxes with no covers. Both covers were in place at the end of the survey.

The maintenance supervisor was present during this observation.
