Based on observation and record review, the provider failed to maintain a protected path of egress from the second floor to the exterior of the building. Two second floor stairways discharged onto the main level and were not provided with a one hour fire resistive enclosure to the exterior of the building. Findings include:

1. Observation at 9:30 a.m. revealed the second floor stairways discharged onto the main level. Further observation revealed a continuous one hour enclosure was not provided to the exterior of the building. Review of the previous life safety code survey confirmed those findings. Those stairways were part of the original building constructed in 1948.
