Based on observation and record review, the provider failed to meet the minimum construction standards of the Life Safety Code. Findings include:

Further observation revealed a continuous one hour enclosure was not provided to the exterior of the building. Review of the previous life safety code survey confirmed those findings. Those stairways were part of the original building constructed in 1948.
