Findings include:
1. On 2/13/12 at 11:10 AM, in the presence of Staff #1, in the morgue, two smoke detectors were found covered with construction caps, disabling them.

Based on observation, it was determined that, the facility failed to ensure Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.
Findings include:
1. On 10/31/11 at 10:10 AM, in the presence of Staff #1, in the endoscopy clean utility room, an ABHR was observed installed above an electrical switch.
