Based on observation and staff interview, the facility did not ensure that electrical wiring and equipment is installed and maintained in accordance with NFPA 70, National Electrical Code. This deficient practice does not ensure prevention of an electrical fire or electric shock hazard, affecting 25 patients and all occupants in 3 of 4 smoke zones. The facility has a capacity of 25 with a census of 6.

During the tour conducted on 9/24/15, between 10:30 am and 2:30 pm, the following is observed:

1.) At approximately 12:35 pm it is discovered that there is a broken electrical outlet cover in Room 108.

2.) At approximately 12:45 pm it is discovered that there is a multiplug adaptor in use in the Front Registration Office Area.

3.) At approximately 1:15 pm it is discovered that there are 3 multiplug adaptors in use in the Emergency Room.

4.) At approximately 1:15 pm it is discovered that there is a broken electrical outlet cover in the Emergency Room.

5.) At approximately 1:20 pm it is discovered that there is an open junction box without a cover on the wall in the Sterilization Room.

Maintenance Staff A was present and acknowledged the findings.

NFPA Standard: Electrical wiring and equipment shall be in accordance with NFPA 70, National Electrical Code, unless existing installations, which shall be permitted to be continued in service, subject to approval by the authority having jurisdiction. 2000 NFPA 101, 9.1.2
