PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation and policy review, the facility failed to maintain a safe environment for inpatient behavioral health residents.

Findings include:

During a facility tour with the Interim Executive Director of Behavioral Health on 2/29/16 at 10:15 AM, the following was observed:
B Hall:
· Activity room with thirteen (13) medium-light weight chairs
· Handicap community restroom containing one (1) medium-light weight chair, non-functioning call light, plastic wall mounted tissue and soap dispensers, and a hand held shower head connected to an approximately four (4) foot tubing.
· Room 129 was unoccupied, but contained a large wheelchair.
· Common television area contained ten (10) medium-light weight chairs
C Hall:
Activity room with fifteen (15) medium-light weight chairs, and one (1) rolling desk chair
Group room with twenty-four (24) medium-light weight chairs.

Rooms 121, 122, 123, 124, 127, 129, and 131 did not contain a call light or device to summon assistance if needed.

The Interim Executive Director of Behavioral Health acknowledged the above findings while on tour, and confirmed that patient rooms were not equipped with an emergency call system.

Review of facility policy RI-01-01, Patient Rights and Responsibilities, effective May 2013, revealed that the patient has a right to be treated in a safe environment. The facility communicates Patient Rights and Responsibilities via:
· Postings at facility Patient entry areas
· Inpatient admission packets
· Facility website
· Closed circuit television
