Based on a tour of the faciliy on January 22, 2019, it was determined that there were multiple locations where fire rating labels on fire rated doors had been painted over or obscured; also fire rating labels on corridor smoke barrier doors were also painted over - staff does complete annual fire door inspections but they had overlooked this requirement.

Based on a review of the facility on January 22, 2019, it was determined that the Women, Infant, Children Center (WIC) contained empty oxygen cylinders not marked to avoid confusion and no segregation between empty and full oxygen cylinders at all locations.
