Portable Fire Extinguishers

Based on observation and staff interview it was revealed the facility failed to ensure the facility's fire extinguishers were installed and maintained no more than sixty (60) inches to the top of the handle in accordance with National Fire Protection Association (NFPA) 10. The facility's census was one hundred eighty-five (185).

Findings include:

1. On 07/26/21 at approximately 11:50 a.m. a tour of the building revealed the facility failed to ensure fire extinguisher number eleven (11), located in Apartment C, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

2. On 07/26/21 at approximately 11:55 a.m. a tour of the building revealed the facility failed to ensure fire extinguishers number fifteen (15) and sixteen (16), located at XA8, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

3. On 07/26/21 at approximately 12:01 p.m. a tour of the building revealed the facility failed to ensure fire extinguisher number seventeen (17), A531 Kitchen, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

4. On 07/26/21 at approximately 12:07 p.m. a tour of the building revealed the facility failed to ensure fire extinguisher number eighteen (18), A537 Kitchen, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

5. On 07/26/21 at approximately 12:11 p.m. a tour of the building revealed the facility failed to ensure fire extinguisher number twenty-three (23), B230 Lab, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

6. On 07/26/21 at approximately 12:15 p.m. a tour of the building revealed the facility failed to ensure fire extinguisher number fifty-seven (57), SF23 Stairs, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

7. On 07/26/21 at approximately 12:25 p.m. a tour of the building revealed the facility failed to ensure fire extinguisher number fifty-nine (59), F162 Stairs, was installed at a height of approximately sixty-five (65) inches to the top of the handle.

8. On 07/26/21 at approximately 12:30 p.m. these findings were verified with the Maintenance Supervisor at the time of discovery, and again with the Administrator at the time of exit.
