Based upon medical record review and interview the facility did not comply with all of the provisions for an appropriate transfer. Please reference findings under Tag A 2409.

Based on medical record review and interview, the facility did not ensure the physician co-signed the certification indicating that the transfer to a higher level of care was appropriate as documented by the mid-level provider in the emergency department (ED) for 5 of 5 transferred patients (Patient # 1, 16, 20, 21 and 22).

Findings include:

Review of medical record for Patients # 1, 16, 20, 21 and 22 revealed no evidence that the physician co-signed the certification indicating that the transfer to a higher level of care was appropriate, as documented by the mid-level provider.

Interview with Staff # 13 on 8/10/16 at 11:30 AM revealed that a physician does not co-sign any documents completed by midlevel providers in the ED. The facility does not have a policy for this requirement.
