Based on document review and interview, the Registered Nurse (RN) failed to ensure the facility policy/procedure for Clinical Assessments was followed for 6 of 10 closed medical records (MR) reviewed (patient # 1, 2, 5, 6, 7 and 8).
Findings include:
1. Policy/procedure, Clinical Assessments, revised/reapproved 4/16 indicated on page 2: "k. Updates and skin assessments will be completely weekly or as needed".
2. Review of patient 1's MR, admitted to facility on 9/20/17 and discharged on 10/10/17, lacked documentation of weekly skin assessments per nursing staff for week of 9/27/17, 10/4/17 and 10/10/17. Review of patient 2's MR, admitted to facility on 8/22/17 and discharged on 9/1/17, lacked documentation of weekly skin assessment per nursing staff for week of 8/29/17. Review of patient 5's MR, admitted to facility on 9/20/17 and discharged on 10/12/17, lacked documentation of weekly skin assessment per nursing staff for week of 9/27/17, 10/4/17, 10/11/17. Review of patient 6's MR, admitted to facility on 9/25/17 and discharged on 10/13/17, lacked documentation of weekly skin assessment per nursing staff for week of 10/2/17 and 10/9/17. Review of patient 7's MR, admitted to facility on 10/6/17 and discharged on 10/19/17, lacked documentation of weekly skin assessment per nursing staff for week of 10/13/17. Review of patient 8's MR, admitted to facility on 10/18/17 and discharged on 10/31/17, lacked documentation of weekly skin assessment per nursing staff for week of 10/25/17
3. On 11/2/17 at approximately 1230 hours, staff P5 (Executive Director) was interviewed and confirmed patient 1, 2, 5, 6, 7, and 8's MR lacked documentation of weekly nursing skin assessments as per facility policy/procedure, Clinical Assessments. Staff P5 confirmed nursing staff should follow policy/procedure for Clinical Assessments.
