The Standard is not met as evidenced by:
Based on interview and policy review, the critical access hospital failed to assure that the Nurse Practitioner members of the staff participated with a Doctor of Medicine or Osteopathy in a periodic review of patient records.
Findings include:
1) Interview with the Nurse Practitioner on duty was conducted. She was asked if the Nurse Practitioners ever sit and review a patients chart, and she stated "no, we don't. The doctors look at the charts, but we don't review them together".
2)-The Policy and Procedure manual was reviewed and there is no policy for review of the charts by the physicians and nurse practioners, and interview with the Chief Nursing Officer indicated that there is no policy for this.

The Standard is not met as evidenced by:
Based on observation, the critical access hospital failed to ensure that all scheduled drugs are current and not outdated.
Findings include:
1) The medication cart on the medical floor was checked for expired medications during tour of the unit. It was found that Carisopordol, 350 mg, was in the cart for patient use and had expired in August, 2011.
2) The locked cabinet of controlled substances on the medical floor was checked for expired medications also during the tour of the unit, and it was found that two Fentanyl Transdermal patches were expired as of August, 2011.
