Based upon observation and interview with staff, the facility failed to provide doors where there are no impediments to the closing of the door. 19.3.6.3.3

The doors to the Gift Shop and the Medical Records were held open by door wedges that impedes the closing of the doors. 19.3.6.3.3

Based upon observations and interview with staff, the facility failed to provide doors to hazardous areas that are held open by a device arranged to automatically close on the activation of the required manual fire alarm and the automatic sprinkler system. 19.2.2.2.6, 7.2.1.8.2
Finding::
The facility failed to provide hold open devices that automatically close upon activation of the required manual fire alarm and the automatic sprinkler system for the Gift Shop Storage Room and the Medical Records Room. 19.2.2.2.6, 7.2.1.8.2

Based upon observation and interview with staff, the facility failed to provide exit signs in all required areas that are displayed in accordance with 7.10 with continuos illumination, 19.2.10.1 Findings

The facility failed to provide an illuminated exit and directional sign on the West side of the opposing swing smoke compartment doors in the main East-West Corridor as required by 19.2.10.1 and 7.10.

In most of the Storage Rooms, the facility failed to maintain continuous and unobstructed areas less than or equal to 18" below the sprinkler deflector that prevent the pattern from fully developing as required by NFPA 13 5-5.5.2.1.
