Based on observation, the facility failed to prohibit penetrations through rated assemblies.

The findings included:

Observation of the first floor mechanical room during the follow up survey on 5/15/15 revealed the following:
A. The ceiling had a penetration around the hot water line that had not been corrected.
B. The air conditioning vent at the rated ceiling had a thin piece of sheet metal around the penetration at the duct and does not give the rating that is required.
C. Three electrical conduits above the hot water tank going through the ceiling has penetrations around each pipe.
D. The duct next to the North wall at the corridor has a penetrations next to it.

These findings were verified by the maintenance supervisor and acknowledged by the administrator during the exit conference on 5/15/15.
