Based on observation, the provider failed to meet the minimum construction standards of the 2000 Life Safety Code (LSC) due to lack of fire protection for the building structure. The maintenance supervisor was aware of the downgraded rating due to the removal of the gypsum ceiling. Findings include:

1. Observation at 2:00 p.m. on 11/16/15 revealed the building was a four story, protected ordinary, Type II (000) structure with a complete automatic sprinkler system. The fire protection for the structure was reduced to that level, because the air compressor/mechanical room ceiling had been removed exposing the steel bar joist structural system.
