Based on review of hospital documentation and interviews, the hospital failed to ensure that infection control practices were followed to ensure that visitors were appropriately screened for symptoms and travel history prior to issuance of a visitor's pass. The findings include:
1. An observation at the hospital's front desk on 9/8/20 at 1:15 PM identified that five (5) visitors, including a surveyor, had their temperatures checked via a thermoscan and were then issued a visitor's pass to enter the hospital. During the observation it was identified that the hospital staff failed to ensure that the entrance screening included a questionnaire for travel and documentation that COVID-19 symptom checks were done.
A tour of the critical care unit (CCU) on 9/8/20 identified one (1) visitor whose screening did not include the questionnaire and symptoms check prior to receiving a visitor's pass. This visitor was observed sitting on a chair visiting a patient in a presumed non-COVID-19 room.
Interview with the Infection Preventionist on 9/8/20 at 2:10 PM identified that visitor screening should include a temperature check, and a questionnaire to ensure that visitors did not have any COVID-19 symptoms or had recently travel to restricted areas.
Review of the hospital visitor policy and screening protocol identified that visitors should be screened for travel, fever, (COVID-19) symptoms, and masked (provided with a face mask). The policy also identified that any visitor or escort who has traveled to a State on the travel advisory list will not be permitted to remain on site.
