Based on observations, document review and staff interview, the hospital had not completed all physical plant construction and/or environmental modifications at the time of the revisit, to ensure a safe environment. The findings include:
During tours of the facility on 5/12/16 it was identified that construction and modifications to the adolescent patient bedrooms (ceilings and lighting) had begun but not yet completed. Half of the unit had newly installed permanent ceilings and appropriate lighting fixtures while the other half remained with the spline ceiling tile system and lighting fixtures deemed not appropriate to a physhiatric setting.
Interview with the Director of Quality on 5/12/16 at 2:45 PM identified that the construction and modifications to the adolescent unit was ahead of schedule and would be completed on or before 5/20/16, as identified in the hospital's initial plan of correction.
Following the completion of construction and modifications to the adolescent unit, the hospital has a plan in place to ensure that the same construction and modifications are implemented throughout the building.
