MAINTENANCE OF PHYSICAL PLANT

Based on document review, manufacturer's instructions review, staff interview, and policy review, the facility failed to ensure the overall hospital environment is developed and maintained in a manner to assure the safety and well- being of patients.


Findings Include:

Review of the facility's temperature logs for the hydrocollator revealed the last temperature check was completed on 3/7/17. Review of the operation manual for the hydrocollator revealed the recommended temperature of the water should be checked with a thermometer after every adjustment, before using the HotPacs. The tank should also be drained and cleaned systematically, at a minimum every two (2) weeks.

During an interview on 05/22/17 at 11:20 a.m. the Rehabilitation Technician stated, "We do temperature checks everyday but do not write them down. We use the hydrocollator 1-2 times a month." The Rehabilitation Technician confirmed the last documented temperature check on the hydrocollator was completed on 3/17/17 and there was no documentation of the hydrocollator being cleaned.

Review of the facility's "Post Over Equipment" policy (not dated) revealed, "... Hydrocollator Tanks once a month or every other month depending on use clean as follows: unplug machine Remove all Hydrocollator packs and discard those that are damaged. Remove all residues and disinfect in diluted bleach solution (ratio 1:9). Rinse thoroughly. Refill to proper level with clean water. Monitor temperature daily and record each day of use. Temperature should be maintained at 165 to 170 degrees Fahrenheit."
