The facility has failed to provide master alarms to the medical gas piping system as required. This could potentially allow for a malfunction of the system to go undetected by the staff. This could place patients, visitors, and staff at risk of a medical gas leaks or other situations.
The findings include, but are not limited to:
Based upon observations and staff interviews during survey rounds on December 16, 2014 between the hours of 11:00am and 2:30pm, while accompanied by the Administration Staff and the Maintenance Director, I observed that the facility has two master alarms, however, one panel alarm was not located in the principal working area of the person responsible for the maintenance of the medical gas piping system. Of the two existing alarms, neither one satisfy the requirement of being in the principal working area of the individual responsible for the maintenance of this system.
This finding was observed and discussed with the Administration Staff and the Maintenance Director.

The facility has failed to ensure the premises is free of electrical hazards. This could allow for an electrical fire to start and thus expose patients, visitors, and staff to the threat of an electrical fire.
The findings include, but are not limited to:
During the survey tour on December 16, 2014, between the hours of 11:00am and 2:40pm, I observed electrical hazards in the following locations:
1. At 11:17am, housekeeping was observed to have refrigerator on power strip.
2. At 11:20am, Kitchen storage desk was observed to have power strip plugged into power strip.
3. At 11:40am, Healthcare Authority Office was observed to have power strip into power strip.
4. At 11:54am, Pharmacy was observed to refrigerator on power strip.
5. At 11:57am, Cardio Pulmonary was observed to have an extension cord on a power strip. This was corrected on site.
6. At 1:44pm, Dr ' s sleeping lounge in ER was observed to have refrigerator on a power strip.
7. At 1:45 ER med room was observed to have 2 refrigerators on power strip.
8. At 2:04 IT was observed to have a refrigerator and microwave on power strip.
These findings were observed and discussed with the Administration Staff and the Maintenance Director that accompanied me during the entire survey.
