Based on observation , staff interview and document review, the facility does not ensure that the two-hour fire door closes completely or latches (Mercy Comprehensive Care Center).
Findings include:
During the facility tour on 06/05/13 at 10:00 AM, it was observed that the two- hour fire door in the diagnostic corridor did not latch when closed. Review of the floor plan for MCCC indicated that this wall is a two-hour rated wall.
This finding was verified with the Director of Facilities and Engineering on 06/05/13.

Based on observation and interview, the facility does not ensure that adequate separation is maintained for the rated walls in 2 of 2 facilities. (Mercy Hospital of Buffalo and Mercy Comprehensive Care Unit (MCCC))
Findings include:
During the facility walk - thru from 06/05/13 thru 06/07/13, it was observed that penetrations were present in the one-hour and two-hour rated walls. Penetrations were observed at the following locations:
MCCC:
Above the ceiling tiles of the medical record storage room (Room #91), the common wall between the mechanical room (Room #94) and the storage room (Room #93) and in the transformer room (Room #150).
This finding was verified by the Director Facilities and Engineering on 06/05/13.
Mercy Hospital of Buffalo:
Above the ceiling tiles in the clean supply room of Pediatrics, in the Soiled Utility Room across from Room #581 and Room #664, ceiling penetrations in Soiled Utility Room #657 and the Clean Utility Room #653.
This finding was verified by Facilities Staff member on 06/06/13 and 06/07/13.
