Based on patient record review, policy and procedure review, and staff interviews, the facility failed to provide quality medical care by failing to follow hospital policy requiring a minimum of one (1) Emergency Department physician on-duty at all times.
Findings Include:
Record review and staff interviews on 10/11/2019 revealed the Emergency Department (ED) physician was not in the facility upon Patient #1's arrival.
On 10/11/2019 at 2:45 p.m. review of the ED CPR (Cardio-Pulmonary Resuscitation) Record revealed that Patient #1 arrived in the ED on 10/8/2019 at 15:40 p.m. and that the MD arrived in the ED at 16:00 p.m.
Interview on 10/11/2019 at 2:10 p.m. with Registered Nurse (RN) #2 revealed, "The EMT asked where the doctor was. I called the doctor and he said he was about 10 minutes away ... he said to continue ACLS until he got there."
On 10/11/2019 at 2:36 p.m., interview with RN #4 revealed, "RN #2 called the doctor who stated he was 10-15 minutes away."
Policy Review on 10/11/2019 at 3:47 p.m. revealed: "Staffing, Bullet Point 1: A minimum one (1) Emergency Department physician is on-duty at all times."
On 10/11/2019 at 3:50 p.m. review of the "Physician Staffing Services Contract", section "TERMS", revealed that the contract to provide Emergency Department Physician Services expired March 31, 2015. No further documentation was offered at exit conference.

Findings Include:
Record review and staff interviews on 10/11/2019 revealed the Emergency Department (ED) physician was not in the facility upon Patient 1's arrival on 10/8/2019 between 15:40 p.m. and 16:00 p.m.
On 10/11/2019 at 2:45 p.m. review of the ED CPR Record revealed that Patient #1 arrived in the ED on 10/8/2019 at 15:40 p.m. and that the MD arrived in the ED at 16:00 p.m.
