PATIENT RIGHTS: CARE IN SAFE SETTING

Based on record review and interview, the hospital failed to provide a safe environment for one of one patient (Patient #9).
Patient #9 had been hospital admitted with a psychiatric history of Post-Traumatic Stress Disorder and Major Depression. On 08/26/22, while receiving care on the hospital's subacute unit, Patient #9 experienced a staff-to-patient altercation and was punched twice on the head by Hospital Personnel E. Administration was unaware of the severity of the incident for over 24 hours.

Findings included:

Patient #9's Discharge Summary dated 09/19/22 at 0823 reflected Patient #9 was admitted on 07/18/22 with diagnoses that included Major Depressive Disorder with Psychotic Features and Post-Traumatic Stress Disorder.

Patient #9's Shift Progress Note on 08/26/2022 at 1742 reflected the patient was " ...involved in physical altercation with staff member ..."

Patient #9's Multidisciplinary Progress Note 08/27/2022 at 1837 reflected an camera footage was reviewed "after reports from other patients that a staff member punched a patient from behind twice .... the severity of the altercation wasn't reported until ...other patients' reports surfaced ...AOC [Administrator on Call] notified."

During an interview on 10/26/22 at 1526 Hospital Personnel C stated that Hospital Employee E admitted to hitting the patient twice.

Hospital Patient Rights' Policy dated 07/17/19 reflected that all patients were granted the right to a treatment environment that ensured "protection from harm ..."
