Based on observation and interview, the provider failed to maintain the electrical system in accordance with the national electrical code in two randomly observed locations (electrical room 1105 and janitor's closet 1058). Findings include:

1. Observation at 9:40 a.m. on 6/17/15 revealed exposed electrical wiring in the electrical room. An electrical conduit coming up from the floor terminated in the electrical room approximately one foot above the floor. A cable was inside the conduit and terminated just outside the conduit. The end of the cable had been splayed separating the wires. The wires were stripped leaving the copper line exposed. It was undetermined if the exposed wires were energized.

2. Observation at 10:05 a.m. on 6/17/15 revealed a janitor's closet near the entrance to the patient recovery area. Further observation revealed a broken ground fault circuit interrupter outlet near the mop sink. The broken outlet exposed the live electrical wires behind the outlet and could create a potential fire hazard.

Interview with the supervisor of plant operations at the time of those observations confirmed those findings.
