PATIENT RIGHTS: ADMISSION STATUS NOTIFICATION

Based on a review of facility documentation, medical records (MR) and interview with staff (EMP) it was determined the facility failed to ensure patients right to have a family member and primary care provider notified of admission to the hospital for 10 of 10 medical records reviewed (MR1, MR2, MR3, MR4, MR5, MR6, MR7, MR8, MR9 and MR10).

Findings include.

A review of facility documentation "Patient Rights and Responsibilities, Reading Health System, revised May 2012, Your Rights:...You have the right to have family member, friend, or support person notified promptly about your admission to the hospital. You have the right to have your personal doctor notified promptly about your admission, and be kept up-to date about your illness and treatment ..."

A review on April 2, 2014, of MR1, MR2, MR3, MR4, MR5, MR6, MR7, MR8, MR9 and MR10 revealed the patients were admitted to the hospital. There was no documentation in the records that indicated that the patients were asked if they wanted a family representative or primary care provider notified of admission to the hospital.

An interview conducted on April 2, 2014, at 10:45AM with EMP3 confirmed that there would not be documentation to indicate the patients were asked if they want their family representative or primary care physician notified of admission because the medical record system does not trigger staff to ask the patients on admission whether they wanted family representative or primary care physician notified of admission.

Interview conducted on April 2, 2014, at 11:15 AM with EMP4 confirmed that patients are not asked if they want their family representative or primary care physician notified of admission.
