Based on document review and interview, the facility failed to ensure adequate numbers of personnel on one unit.
Findings include:
1. Review of Staffing Guidelines/schedules for the medical/surgical unit indicated the following. When unit has a census of 28 (12/13/2019) on 3p to 11:30p (1500 hours to 2330 hours) unit should have had 6 RN/LPNs, (registered nurse/licensed practical nurse) unit had 5 RN/LPNs. When unit has a census of 27 (12/14/2019) on 7a to 330 p (0700 hours to 1530 hours) unit should have had 6 RN/LPNs, and unit had 5 RN/LPNs.
2. Interview on 1/28/2020, at approximately 1427 hour, with N7 (Administrative Director Patient Care Services) confirmed staffing grid indicated 6 nurses for 27 patients, and they only had 5 nurses.

Based on document review and interview, the facility failed to ensure standards of nursing care were provided for 4 of 10 patients (patient 2, 5, 6, and 8).
Findings Include:
1. Review of patient 2 (admission of 12/04/2019 through 12/24/2019), 5 (admission of 12/12/2019 through 01/03/2020), 6 (admission of 12/05/2019 through 12/20/2019), and 8's (admission of 12/13/2019 through 1/3/2020) MR indicated changes in the intervention of bed alarm, put in place and discontinued without documentation of indication for changes.
2. Interview on 1/29/2019, at approximately 1418 hours, with N4 (Chief Nursing Officer) confirm patients 2, 5, 6, and 8's MRs lacked documentation of indication for bed alarm use and discontinue of use.
