Based on record review and interview, the hospital failed to enforce it's qualifications and competence policy that required orientation of nurses and other personnel who provide nursing care to patients for 2 of 4 (Personnel #5 and #11) personnel, in that, Personnel #5 and #11 did not have orientation to the Inpatient Rehab Unit prior to their assignment to care for patients.

Findings Included

Personnel #5's and #11's file did not include orientation to the Inpatient Rehab Unit prior to their assignment to care for patients.

During an interview and personnel file review on 12/08/2015 ending at 1:07 PM, Personnel #13 was asked for the Inpatient Rehab Unit orientation for Personnel #5 and #11. Personnel #13 stated, "There is not (for either person) one."

The hospital's 04/01/2015, effective date "Employee Orientation new rehabilitation staff" policy required, "department specific orientation process...provide a comprehensive orientation...completed skill checklist will be maintained...review of job description...All staff must complete orientation prior to fully participating in the patient care process...All shared staffing employees will be oriented to the Rehab Unit..."
